Pathologic 3 is a 2026 survival horror RPG developed by the Russian studio Ice-Pick Lodge and published by HypeTrain Digital. A standalone sequel and reimagining of the Bachelor route from the cult classic Pathologic, it follows the arrogant idealist doctor Daniil Dankovsky as he is drawn into a remote, decaying town besieged by the lethal sand plague during a twelve-day catastrophe. The game recasts the survival loop around forensic plague investigation and a two-run Twin structure in which knowledge from a first playthrough reshapes the second, letting players master time, medicine, and the town's trust to trace the epidemic to its source. Set in the isolated Town-on-Gorkhon and the surrounding Steppe, it retains Pathologic's celebrated systemic quest web, name-driven NPC schedules, and oppressive atmosphere of communal collapse. A prologue titled Pathologic 3: Quarantine was released in March 2025.

  • Day-and-Night Clock Survival - Each in-game day runs on a real-time schedule; quests from named NPCs arrive over the course of the day and are erased at midnight, while each day's main mission decides whether a key character (one of the Bound) lives or dies.
  • Name-Driven Quest Web - Rather than scripted chain quests, objectives are generated by individual townsfolk whose movements follow the clock; who is alive, who remembers the Bachelor, and which secrets are revealed all depend on completed and failed interaction.
  • Community & Trust Management - The Bachelor must negotiate favor, information, and supplies against the town's deteriorating social order, choosing which factions, families, and factions' interests to serve as the epidemic consumes the settlement.
